PKC Access Team based at Letham Hub

Set up a Social Work Access Team at Letham4all to allow locals to pop into office to discuss care concerns. They've a washing facility there too with hoists. Maybe catch the early problems before a crisis occurs eg neck alarms, shower handles, personal care 

Because it's about care in the community and trying to prevent crisis situations. People need to know where to start accessing care.
